The HCV IgM test is recommended for individuals exhibiting symptoms of liver dysfunction—such as jaundice, fatigue, or abdominal pain—or those with elevated liver enzymes. It is also useful in cases of suspected acute hepatitis C, reactivation of chronic HCV, or when distinguishing false-positive results from true infections following an initial antibody screening. High-risk groups include healthcare workers, intravenous drug users, individuals with multiple sexual partners, and patients undergoing dialysis or organ transplants.

This test involves a simple blood draw, typically performed at a diagnostic center or via home collection services. No fasting is required, and the sample is processed to detect the presence of IgM antibodies specific to HCV. Patients are advised to avoid heavy meals, caffeine, and nicotine before the test to ensure optimal sample quality. If the patient is on medication that could affect immune response, it should be disclosed to the healthcare provider. Results are usually available within 24 to 48 hours, depending on the lab’s processing time.

A positive HCV IgM result suggests recent or ongoing infection and warrants further testing with HCV RNA (PCR) to confirm viral presence and load. If IgM is detected but RNA is absent, it may indicate a resolved infection or false-positive result. Conversely, a negative IgM result typically rules out recent infection, though follow-up testing may be necessary if recent exposure is suspected. Accurate interpretation requires correlating IgM findings with clinical symptoms, liver function tests, and confirmatory RNA assays to guide treatment decisions.
