Most Americans who have “isolated diastolic hypertension” today got it without a single millimeter of mercury changing inside their arteries. A committee moved a line.+

Same arteries. Same people. New label. That fact explains most of the contradictory advice you will find about your bottom number.

Quick Answer: High diastolic pressure means the bottom number of your reading is 80 mm Hg or higher. When the top number stays under 130, doctors call it isolated diastolic hypertension. It affects roughly 6 to 9 out of every 100 US adults, most often people under 55. A reading of 80 to 89 usually calls for lifestyle changes and repeat monitoring, not immediate medication. Readings of 90 or higher deserve a medical evaluation.

What Counts as a High Diastolic Number (And What Changed)

What the bottom number actually measures

Your heart spends roughly two-thirds of every cardiac cycle at rest, refilling. Diastolic pressure is the force still pressing on your artery walls during that resting phase.

The top number reflects how hard your heart pushes. The bottom number reflects how much your small arteries resist. Those are two different physical problems with two different sets of causes.

That distinction matters. A high bottom number with a normal top number is not “mild hypertension.” It is a separate pattern with a separate mechanism.

The current US categories

The 2025 AHA/ACC High Blood Pressure Guideline, released in August 2025, sets the framework every US clinician now works from.

Normal is under 120 systolic and under 80 diastolic. Elevated is 120 to 129 systolic with diastolic still under 80. Stage 1 hypertension is 130 to 139 systolic or 80 to 89 diastolic. Stage 2 is 140 or above systolic or 90 or above diastolic.

Notice the word “or.” A reading of 118/86 counts as stage 1 hypertension under current US rules, even though the top number sits comfortably in the normal range.

Why the 80 mm Hg line is newer than most people realize

The diastolic cutoff sat at 90 mm Hg for decades. The 2017 guideline lowered it to 80, and its authors were candid that the new threshold rested on expert opinion rather than trial data.

Johns Hopkins researchers measured what that decision did. Analyzing 9,590 adults from NHANES, they found the estimated prevalence of isolated diastolic hypertension was 6.5% under the 2017 definition and 1.3% under the older one, an absolute difference of 5.2 percentage points.

Here is the part almost nobody quotes. Among everyone newly reclassified, only about 0.6% also met the guideline own threshold for starting blood pressure medication.

A meaningful share of “high diastolic” readings are measurement artifacts. Small positioning errors move the bottom number by 4 to 6 mm Hg, which is the entire distance between 82 and 88.

The arm-position figures come from a 2024 crossover randomized trial in JAMA Internal Medicine (Liu et al., 2024;184(12):1436-1442) that tested 133 adults across three arm positions. The cuff-size numbers come from the Cuff(SZ) trial published in the same journal a year earlier.

The mechanism, in one paragraph

Blood pressure is cardiac output multiplied by vascular resistance. When the top number climbs while the bottom stays put, the problem is almost always resistance rather than output.

Your arterioles are constricting more than they should, which keeps pressure elevated even while the heart rests. That is why the drivers here skew metabolic and neurohormonal rather than age-related.

Excess weight and insulin resistance

This is the single most common driver in adults under 55. A 2026 review in Frontiers in Cardiovascular Medicine mapped the pathway in detail, describing an insulin resistance to systemic vascular resistance to isolated diastolic hypertension axis.

The mechanism runs three ways at once. Insulin resistance raises sympathetic nerve activity, blunts nitric oxide release from the endothelium, and preserves insulin sodium-retaining effect on the kidney even as its glucose-handling effects fail.

The review names three practical markers that track with this pattern: HOMA-IR, the triglyceride-glucose (TyG) index, and METS-IR. All three can be calculated from a standard fasting panel.

Sodium and salt sensitivity

Sodium raises diastolic pressure through volume expansion and direct effects on vascular smooth muscle. Salt sensitivity varies widely, and it is more pronounced in Black Americans, older adults, and people with reduced kidney function.

The practical target most US clinicians use is under 2,300 mg per day, with 1,500 mg as the ideal for people already hypertensive. The average American adult consumes roughly 3,400 mg, and about 70% of that arrives in packaged and restaurant food rather than the salt shaker.

Alcohol

Alcohol is the most reversible cause on this list, and the most underestimated. Regular intake raises both numbers, with the diastolic effect appearing at lower intakes than most people expect.

Cutting back produces measurable change within two to four weeks. That short timeline makes alcohol a useful variable to test during a home monitoring period.

Obstructive sleep apnea

Sleep apnea appears in an estimated 25% to 50% of people with resistant hypertension. Each apneic episode triggers a sympathetic surge, and repeated hundreds of times per night, those surges reset baseline vascular tone.

The signature is a bottom number that stays high overnight and early in the morning. Loud snoring, witnessed breathing pauses, and daytime sleepiness together justify a sleep study.

Stimulants, decongestants, NSAIDs, and hormonal contraceptives

Pseudoephedrine and phenylephrine constrict vessels systemically, not just in your nose. NSAIDs including ibuprofen and naproxen raise pressure by promoting sodium retention and by blunting the effect of several blood pressure medications.

Prescription stimulants for ADHD, some antidepressants (particularly SNRIs), corticosteroids, and combined oral contraceptives all shift diastolic pressure upward in susceptible people, as do many energy drinks and preworkout formulas.

Kidney disease

The relationship runs in both directions. Reduced kidney function raises pressure through sodium retention and renin activation, and sustained high pressure damages the kidney further.

Thyroid dysfunction

An underactive thyroid raises peripheral resistance and lifts the bottom number while leaving the top alone. That makes hypothyroidism a classic reversible cause of isolated diastolic hypertension.

An overactive thyroid does the opposite, raising the top number and widening pulse pressure. A single TSH test separates the two.

Primary aldosteronism

Long treated as a rare curiosity, primary aldosteronism now appears in an estimated 5% to 25% of people with resistant hypertension. The 2025 guideline widened who should be screened.

Screening is now recommended for resistant hypertension with or without low potassium, unexplained low potassium, obstructive sleep apnea, an incidentally discovered adrenal mass, a family history of early-onset hypertension, or stroke before age 40.

What does not explain it

Stress alone does not cause sustained diastolic hypertension, though it raises readings acutely and can skew a monitoring week. Dehydration typically lowers blood pressure rather than raising it.

A single high reading explains nothing. Blood pressure varies by 10 to 20 mm Hg across a normal day, and diagnosis requires an average of at least two careful readings on two separate occasions.

How Much Does a High Diastolic Number Actually Raise Your Risk

This is where honest reporting gets uncomfortable, because researchers have spent six years disagreeing in print.

The case that it barely matters

The Johns Hopkins group followed 8,703 adults in the ARIC cohort for a median of 25.2 years. Isolated diastolic hypertension, defined by the 2017 threshold, showed no significant association with atherosclerotic cardiovascular disease (1,386 events, HR 1.06, 95% CI 0.89 to 1.26).

Heart failure was also null (HR 0.91, 0.76 to 1.09). So was chronic kidney disease (HR 0.98, 0.65 to 1.11).

They then checked two more cohorts. Cardiovascular mortality hazard ratios came back at 1.17 in NHANES and 1.02 in CLUE II. Neither reached significance.

Those same researchers later published a formal “con side” argument in Hypertension, making a point that still stands. No clinical trial has ever enrolled only people with isolated diastolic hypertension and shown that treating them helps.

The case that it matters a great deal

Kaiser Permanente Northern California pulled more than 36 million outpatient blood pressure readings from 1.3 million adults between 2007 and 2016, tracking 44,286 heart attacks and strokes across eight years.

Both numbers independently predicted events. Systolic burden carried the larger effect, but diastolic burden held up on its own after adjustment for demographics and comorbid conditions. Decades of teaching had pushed the bottom number aside, and the data did not support that.

What the newest research settled

Two studies published since late 2025 changed the conversation. The first pooled 51 randomized controlled trials covering 358,325 participants, 15,845 of whom (4.4%) had isolated diastolic hypertension. Published in the European Heart Journal, it asked whether blood pressure medication actually works in this group.

It does. Over a median 4.2 years, a 5 mm Hg systolic reduction cut major cardiovascular events by an almost identical amount in people with isolated diastolic hypertension (HR 0.91, 95% CI 0.82 to 1.01) and without it (HR 0.90, 0.89 to 0.92), with a P value for interaction of exactly 1.00.

Event rates in the isolated diastolic hypertension group fell from 114.7 per 1,000 in the comparator arm to 101.3 per 1,000 in the treatment arm.

The second study, published in Hypertension in August 2026, pooled four US cohorts totaling 23,957 adults with 2,320 atherosclerotic events and 1,243 heart failure events over a median 17.2 years.

Among young adults aged 18 to 39, isolated diastolic hypertension carried an adjusted ASCVD hazard ratio of 1.36 (95% CI 1.11 to 1.68) and a heart failure hazard ratio of 1.69 (1.24 to 2.31), compared with normal blood pressure.

The progression data may be the more useful finding. Among young adults who started with isolated diastolic hypertension, 29.8% still had that pattern later in life, and 36.5% had progressed to combined systolic-diastolic hypertension.

Putting the four findings together

Four studies, apparently four answers. They reconcile more cleanly than they look.

The ARIC null result came from a cohort with a mean baseline age of 56, where arterial stiffening was already underway and the bottom number added little. The positive young-adult result came from a population where it adds a lot.

The Kaiser data confirms both numbers carry independent signal at population scale. The trial meta-analysis confirms that if treatment is warranted for other reasons, it works normally in this group.

So the practical reading is this. A high bottom number at 35 is an early warning about metabolic and vascular health with a one-in-three chance of becoming full hypertension. The same number at 60 adds less independent risk but raises the question of what else is going on. Neither situation calls for panic, and both call for a plan.

The J-curve

Your coronary arteries fill during diastole. Push the bottom number too low and you can compromise blood supply to the heart muscle itself, even while the top number looks textbook.

Researchers examined 11,565 ARIC participants to test whether this happens in practice. Compared with a diastolic pressure of 80 to 89 mm Hg, the adjusted odds of having elevated high-sensitivity troponin T (14 ng/L or above) were 2.2 times higher at diastolic under 60 and 1.5 times higher at 60 to 69.

Low diastolic pressure also tracked with progressive troponin rise over the following six years, and with incident coronary heart disease and death across 21 years of follow-up.

The stroke result is the tell. Low diastolic pressure showed no association with stroke, which is exactly what you would predict if the mechanism is coronary perfusion, since brain blood flow continues through both phases of the cardiac cycle.

Troponin is among the tests our lab partners run most often, and this finding is a reminder that it can flag slow, silent strain rather than one dramatic event.

The counterpoint from trial data

Observational data and trial data disagree here, and both deserve weight.

The 2026 European Heart Journal meta-analysis found that relative risk reductions from blood pressure lowering did not shrink in people whose baseline diastolic pressure was already low, including below 60 mm Hg.

Those are different questions. The ARIC finding is about where your diastolic pressure sits naturally. The trial finding is about whether lowering it further causes net harm. Current evidence suggests untreated low diastolic pressure with a wide pulse pressure signals underlying disease, while supervised treatment does not appear to create that risk.

The practical floor

The ARIC authors own recommendation was measured. When treating toward a systolic target, it is prudent to keep diastolic pressure from falling below 70, and particularly not below 60.

This applies mainly to older adults, people with coronary disease, and anyone with a pulse pressure above 60 mm Hg. If you are 35 with a reading of 118/86, it is background rather than a personal concern.

What to Do About It, Step by Step

Step one: confirm it properly

Do not act on office readings alone, and do not act on one home reading either.

Run a seven-day log. Two readings each morning before medication or coffee, two readings each evening, taken one minute apart, seated with your back supported, feet flat, arm on a table at heart level, after five minutes of quiet rest.

Discard day one, since first-day readings run high. Average what remains. That number, not your worst reading, is what your clinician needs.

Step two: the tests worth asking for

A high bottom number is a prompt to look for a cause, and most of the useful causes show up on standard lab work.

Ask about a basic metabolic panel with eGFR and potassium, a urine albumin-to-creatinine ratio, TSH, a lipid panel, and HbA1c or fasting glucose. If you meet the 2025 screening criteria, add an aldosterone-to-renin ratio.

Patients commonly ask us whether all of this is needed at once. It usually is not. The metabolic panel and TSH most often change the plan, and the rest can follow if those come back clear.

What this costs in the United States

A validated upper-arm home monitor runs $40 to $80. Cuffs for larger arms are a $15 to $25 add-on, and the right size matters more than the fancier model.

A basic metabolic panel typically costs $10 to $40 cash-pay, TSH $20 to $50, lipid panel $15 to $50, and HbA1c $15 to $40. Bundled cardiac and metabolic packages often price below the sum of their parts.

Most commercial plans and Medicare cover these when ordered for a documented reason such as elevated blood pressure. Blood pressure screening carries a Grade A recommendation from the US Preventive Services Task Force, so most ACA-compliant plans cover the screening visit without cost sharing.

An aldosterone-to-renin ratio runs $100 to $250 and is worth ordering only when the screening criteria genuinely apply. A home sleep apnea test costs $150 to $500 out of pocket and is widely covered when symptoms are documented.

Step three: the changes with the strongest diastolic effect

Sodium reduction toward 1,500 to 2,300 mg daily, which for most people means cutting packaged and restaurant food rather than cooking with less salt.

Weight loss, where even 5% to 10% of body weight produces measurable change. For a 200-pound adult that is 10 to 20 pounds, not a transformation.

Alcohol reduction, which shows results fastest of anything on this list.

Aerobic exercise at 150 minutes per week of moderate activity, matching the federal physical activity guidelines. Add isometric work, which has an outsized effect on diastolic pressure specifically: four sets of two-minute wall sits or handgrip holds at moderate effort, with two minutes of rest between sets, three days a week.

Treatment of sleep apnea, which can hold the bottom number up regardless of everything else you do.

Adequate potassium, with a general target of 3,500 to 5,000 mg daily from produce, beans, and dairy. This one carries a hard exception. Anyone with reduced kidney function or taking a potassium-sparing medication should not raise potassium intake without clinician direction.

The DASH eating pattern combines most of these levers and remains the best-tested dietary approach for blood pressure in US populations.

Step four: when medication enters the conversation

Under the 2025 AHA/ACC framework, the path depends on your calculated risk.

For adults with blood pressure at or above 130/80 and lower cardiovascular risk, defined as under 7.5% by the PREVENT equation, medication is recommended if three to six months of lifestyle change fails to bring the reading under 130/80.

Higher risk, established cardiovascular disease, diabetes, or chronic kidney disease moves the timeline forward. Stage 2 readings of 140/90 or above generally warrant medication now, typically as a single-pill combination of two agents.

First-line agent selection under the 2025 guideline is race-neutral. Without a compelling indication for something else, that means a thiazide diuretic, an ACE inhibitor, an ARB, or a dihydropyridine calcium channel blocker.

The guideline own caveat deserves repeating. Its summary of remaining knowledge gaps names management of isolated diastolic hypertension, more common in younger populations, as an area where evidence remains insufficient to define clear treatment thresholds and targets.

Your twenties and thirties

This is where isolated diastolic hypertension is most common and most consequential. The four-cohort 2026 analysis gives you a number to act on: roughly one in three young adults with this pattern progresses to both numbers being high.

Treat the reading as an early signal about metabolic health rather than a heart problem. Our medical reviewers note this age group is also the least likely to have had a blood pressure check in the past year.

Pregnancy

Blood pressure rules change during pregnancy, and general adult guidance does not apply. New elevated readings after 20 weeks, sustained readings at 140/90, or any reading at 160/110 require same-day contact with your obstetric team.

If your bottom number stays high on treatment while the top number is controlled, flag it specifically. That pattern can point to an untreated secondary cause, an interfering medication, or a drug class mismatched to a resistance-dominant problem.

Narrowing or widening pulse pressure

Pulse pressure is the top number minus the bottom. A normal range is roughly 40 to 60 mm Hg.

A widening gap, common after 60, points to arterial stiffening. A narrow gap under 25 mm Hg, especially if it appears suddenly with symptoms, can signal reduced cardiac output and warrants urgent assessment.

Athletes and slow resting heart rates

A slow heart rate lengthens diastole, which can nudge the bottom number upward without indicating disease. Well-trained endurance athletes sometimes show this pattern. It does not exempt you from monitoring, but it argues for reading borderline numbers in context.
